Health Forms

All scouts, and any adult who attends a scouting campout or offsite activitiy, must have completed a health form. Download the 2021 forms at: https://filestore.scouting.org/filestore/healthsafety/pdf/680-001_abc.pdf

Required:

• Part A form
• Part B1 and B2 form
• Part C form (this physical signed and dated by your provider MUST be completed on this form)
• copy of your current vaccinations
• copy of front and back of your insurance card
